– Authorization Login Code provided by a Berger Foundation Board Member
– Name of the Berger Foundation Board Member who referred your nonprofit organization
– 1-2 Page Grant Request Letter (include requested grant amount, description of program, date, signature)
– IRS 501(c)3 Determination Letter
– Pages 1-2 of Most Recent 990 or 990-EZ Tax Return
